To request disclosure of electricity charges, usage, etc., please apply using the "Request Form for Disclosure of Electricity Charges and Usage Records, etc."

b. Method for Confirming the Identity of the Individual or Their Agent

Confirm your Identity
If you are requesting in person, please enclose copies of your driver's license, health insurance card, passport, resident registration card, etc. with your request.

Confirmation of the Agent's Identity
If a legal proxy or an agent entrusted by the individual is making the request, please enclose the following documents with your request, in addition to the documents for individual verification in item a. above. ([a] is a copy, [b] is the original)

(a) For a Legal Representative
A certified copy of the family register or other documents that prove legal proxy rights, and documents for verifying the identity of the legal representative (such as a driver's license, health insurance card, passport, or certificate of residence).

(b) For an Agent by Entrustment
A letter of attorney sealed with the individual's personal seal. (If necessary, we may ask for the submission of a registered personal seal and a certificate of seal registration.)

We will, in principle, provide our response in writing (including electronic data). However, we may also respond verbally with your consent.
Please be aware that in the following cases, we may not be able to fulfill your request, and we kindly ask for your understanding in advance.

a. If you do not follow the procedures regarding the specific procedures for disclosure, etc., as specified by our company in（3）（1） above.
When it is not possible to confirm the identity of the individual or agent by the method set out in（3） ① b.Specific procedures for disclosure, etc. c. If the request is for the disclosure of information of a person other than the individual. d. In other cases where there is a valid reason based on laws and regulations, etc.

A prescribed commission may be charged for requests for the notification of purpose of use or the disclosure of retained personal data.
